Indiegogo backers do need to pay shipping fees using the survey form.
This was stated at the top of our campaign page:

We opted out of using Indiegogo's shipping rate calculator, so by default Indiegogo's system lists each country as “Free Shipping”, even though this is not the case.
We are using our own shipping provider’s rate calculator because it is typically less expensive for backers to ship multiple Perks together in one package than to charge a base shipping fee for each item separately. We have also designed the order form so that backers may change their order at any time, and we decided this is the best way to pass on the savings to you.
